When emergency responders arrived, they discovered a man, identified as 64-year-old Timothy J. Hennen of rural Morton, at the bottom of an embankment along the Birch Coulee Creek. Both bystanders and emergency responders attempted life saving measures and resuscitation efforts, however Hennen was pronounced dead at the scene.

Preliminary investigation reveals Hennen was riding on horseback, along the top edge of the creek when the horse lost its footing and fell, causing Hennen to fall from the horse.
