MARSHALL — Avera@Home Southwest Minnesota, based in Marshall, Minn., has been awarded a 5-star rating by the Centers for Medicare & Medicaid Services (CMS), the highest rating given to hospice agencies.
CMS evaluates hospice providers quarterly based on survey feedback from patients’ family members and caregivers. The ratings, which range from one to five stars, are published on the Care Compare website to assist consumers in comparing services. This is the first time Avera@Home Southwest Minnesota has received a 5-star rating.

The updated ratings were released on Feb. 19, 2025, using data from the Consumer Assessment of Healthcare Providers and Systems (CAHPS) Hospice Survey, which evaluates communication, symptom management, caregiver support, and overall experience.
Avera@Home Southwest Minnesota serves a 16-county area in southwest Minnesota from its offices in Marshall and Worthington.
